The game has graphical issues in fullscreen on higher resolutions due to broken scaling in Adobe Flash I'd assume. A possible workaround for this issue would be the launch options mentioned above. These commands only work in a KDE environment, but I'd assume you can achieve the same result in other environments with a similiar set of commands.
- It temporarily decreases the screen resolution correctly before running the game.
- It starts the game via %command%
- It readjusts the resolution and refresh rate after the game quit.
Of course this command can't just be copy & pasted for your setup. You need to get your current display resolution, your refresh rate and your display id via 'kscreen-doctor -o'. Once you got that info you can replace those parts in the third command. The shell commands are seperated via semicolon.

Fullscreen does not work in this game. It works on Steam Deck (or Gamescope session) though.
Unlike on Windows fullscreen covers the entire height of my screen but moving objects have visible bounding boxes. Windowed mode is normal.

Cloud saves don't work very well cross-platform.
Steam Cloud work fine on Windows PCs. On SteamOS/Linux - hit or miss (like in my experience - Epic Fetus challenge carried over just fine, but Polaroid unlock status didn't). Make sure to backup the following files:

Additionally, limit screen's refresh rate to 30 Hz. And select community config for controls. The default config is gamepad controls, which don't work.

Game works just fine but...
It becomes very laggy after playing like an hour
There is already no point of buying this game in 2024 since there is a better version of it on the Steam Store but if you want to play and complete this game make sure you restart the game after an unsuccessful run because it becomes very laggy if you play for more than an hour. If you want to get all the achievements, do not waste too much time in the Womb/Utero levels. Skip to the Cathedral and Chest levels asap.

Fun game, runs perfectly

Imported my cloud save just fine and runs just as I would expect it to on Windows.
This is a 12 year old Flash Game. It's expected that it has issues and needs some tweaks to get working.
Locked to 30FPS (this was more to keep the frames slightly more stable than to save battery, but I couldn't find another section to put this in.)
Game doesn't fullscreen by default, though all that adds is borders.
Game's Steam Cloud is apparently non-functional, so Backup your save file before uninstalling just in case.
Only pressing Space (L2) skips the opening scene for some reason, no other buttons work.

Save data is deleted when the game is uninstalled. No Steam Cloud support.
Moslty due to the game's nature, flash games aren't meant to be this big and this causes any machine to sometimes struggle.
Unfortunately this game wasn't meant to be played with a joystick which sometimes causes diagonal movements to be a bit wonky and even charge attacks like Brimstone to not shoot properly, also make sure to not take many "speed up" upgrades because it makes Isaac impossible to control.
Added some other keybinds to make the play experience better
After playing for an hour or so, the game takes >90% of cpu for no reason and the game skips A LOT of frames when u try to play

In the beginning the game didn't even start. If you are a manjaro/arch user try using a steam-native version instead of steam-runtime. This made the trick for me. Also if it happens that you have a dual GPU you might add DRI_PRIME=1 %command% on properties -> launch options.

Playing with sticks or pads might be a bit wonky. Use the physical buttons for better accuracy. Consider remapping some buttons.
The game works out of the box but the default controller mapping results in no input, change the controller profile to play the game successfully
The game runs great apart from switching the control layout, which takes a couple of seconds at most
By default, the firing keys are bound to the left analog stick; I recommend changing them to the right analog stick.
