Replace files with PLAZA crack ("BlackOps.exe", "steam_api.cdx", "steam_api.dll", "steam_emu.ini" Need to play offline because of crack
No multiplayer or zombies. No achievements. If you don't disconnect from internet (or block via firewall) the game won't start [DNS error]
Another case of the well known DRM issue
You need to replace iw4sp.exe with the cracked version
Single player works Off-Steam (no achievements or CO-OP)
Note that with SP cracked you lose achievements and coop but you can still play MP without problems

I tried with cracked iw5sp.exe but crashes on start with "exceeded limit of 128 'leaderboarddef' assets" error
I used the crack from PLAZA
Sometimes when loading new mission crashes with directx error

Eternal loading times, sometimes crashes when loading
I managed to play it with Steam in offline mode and game in safe mode
