Technically starts and runs, but it's totally broken. First time I booted the game, it crashed almost immediately at the Capcom logo. Second time I booted it, it started up properly, but my controller inputs were acting on their own, completely randomly. Then the game crashed after less than a minute.
Disable Uplay in-game overlay and notifications, and the game works pretty much perfect. Performance not quite as good as in Windows as usual, but still perfectly smooth and playable on my machine. Solid Proton title.
Played through the entire game. Set PROTON_FORCE_LARGE_ADDRESS_AWARE=1 %command% in order to avoid crashes and allow MSAA higher than 2x without the screen blanking out. Performance is worse than in Windows, with noticeable framerate dips in some areas, but very playable and still better than the console version.
Crashes immediately on Proton. Runs with Lutris method, but performance is a lot worse than WIndows and there are graphical glitches.
It should be noted that KOTOR usually does not work out of the box even on modern Windows, it is not an issue exclusive to Proton. The same steps taken to get the game working properly at a modern resolution and fullscreen on WIndows (documented in this video: https://www.youtube.com/watch?v=9SKMzwKE34E) work equally well on Proton. Some steps need to be adapted (You must rename swconfig.exe and unwis.exe to swkotor.exe and run through them Steam, or just run them normally through wine like I did). After that, the game runs and looks perfect.
